Service guide

Tell us what you are asking for, or ask us to help you choose.

Ten questions produce a suggested route, with the reasoning shown. A suggestion is not a diagnosis, what you requested is kept on record, and nothing is opened until you and a clinician confirm the service.

Part 1 — What you are asking for

Which service are you asking for?

If you name a service, it is preserved on your request exactly as you asked for it. The Check-Up is not required before any other service. Either way, a person confirms the final service assignment.

Part 2 — What you want and what you are seeing

1. What outcome do you want from this?
2. What symptoms are you seeing now?

Select everything that applies. Record only what you have observed.

3. How urgent is it?

Part 3 — Scope, funding and monitoring

4. How many businesses or entities are in scope?
5. Is funding or a due-diligence process involved?
6. Do you need agreed measures monitored over time?

Part 4 — Immediate risk, specialists and evidence

7. Do any of these apply right now?

These describe immediate business risk. Select “None of these apply” if none do.

8. Do you already know a specialist field is needed?
9. How ready is your evidence?

This affects sequencing only. Missing evidence is recorded as missing, never estimated.

10. How should we contact you?
